Transitions of Care Health Fair Slated October 17
Bartlesville, Okla. (September 9, 2015) -- The Transitions of Care Health Fair is planned for Saturday, October 17 from 9 a.m. to 1 p.m. at the Bartlesville Community Center.
“The purpose of the health fair is to come together as one community with one heart to educate, create an awareness of local healthcare related resources, and share how local providers are working together to implement the best care across the continuum,” said JPMC Medical Staff QI Coordinator and Community Care Transitions Team Member Anna Cherry.
The fair is targeted to seniors and will feature a wide array of health and healthcare-related exhibit booths and displays, door prizes and entertainment. Screenings include blood pressure and blood sugar, back health checks, and hearing tests. Flu shots are available for $25 or free with proof of Medicare. ) and door prizes
Several departments from Jane Phillips Medical Center will participate including Jane Phillips MedCare Pharmacy, Emergency Services, Imaging Services, Mammography, Sleep Center, Case Management, Behavioral Services, and Physical Medicine.
Various community organizations/businesses will be on hand to share their services and provide educational information. These include AdamsParc, Amedysis, Bartlesville Health & Rehab, Hearing Life, Heritage Villa Nursing Center, Legal Aid of Bartlesville, Medicallodges of Dewey, Precision Spinal Health Care, Rivercross Hospice,
Shanadoa Home Health, Tallgrass Estates, The Journey Home, Visiting Angels of Green Country, Washington County Association for Mental Health, Washington County Health Department, Washington County Suicide Prevention, Access Medical, Air Evac Lifeteam, All Saints Home Medical, Green Country Village, Comforting Hands Hospice, Wound Care Specialists, and Carter Healthcare.
Community Care Transitions Team is a non-profit team of healthcare providers who work together to improve the patient’s transition between the hospital and the next level of care.
