Mission Statement

The mission of the Palliative Care Team at Jane Phillips Medical Center is to improve the quality of life for those patients and families admitted with a serious life-threatening or life-limiting illness. Our team of skilled multidisciplinary professionals provides compassionate care, focusing on patient-established goals. The Palliative Care Team will preserve the dignity of our patients and families, and aggressively strive to alleviate their physical, spiritual, psychological, and social suffering.

Criteria for Referral

  • Any serious or life-threatening condition
  • New diagnosis of life-limiting illness
  • Difficult to control physical or emotional symptoms

  • Palliative care registered nurses
  • Social worker
  • Registered dietitian
  • Staff nurse
  • Attending physician
  • Clinical nurse specialist
  • Patient relations associate
  • Pastoral care
  • Pharmacist
  • Additional consults as necessary

The Palliative Care Team will work with the patient’s physician to optimize symptom control and promote the highest quality of life possible for the patient and family by utilizing evidence-based care guidelines.

After a referral is received, the Palliative Care Team will meet with the patient and family to assess their needs, establish the patient’s goals, and develop a plan of care based on those goals. Our team will collaborate with the attending physician to implement the care plan and inform the patient and family about appropriate care options to help meet their goals.
